Applications for grants are comprised of two sections - Organization and contact information, and Proposal Information. Applications should be no more than 3 pages in length.
Organization and Contact Information
Organization name
Contact person name, address, telephone number, web-site and email address
Contact person's role in the organization
Tax Exempt Status
Please attach a copy of your IRS Letter of Determination indicating 501(c)3 tax exempt status.
Proposal Information
- Amount requested
- Brief description of the project (20 words or less)
- Description of the problem to be solved
- Objective(s)
- Strategy to achieve objectives
- How does this project or program help fulfill the objectives of the Bessie Minor Charitable Fund?
- How will this project or program contribute to the community?
- How will you evaluate the results of this project or program?
- Budget, and other sources of funding
- Timetable for the project
- Other comments
Reporting Requirement
We would like grant recipients to report their progress to the Fund six months after the grant is received. Please include the following in that report:
- Recipient of the Grant
- The dollar amount of the grant, including allocation between programs, if multiple are involved
- Specific detail regarding use of the funds
- Effect of the Bessie Minor Grant on the organization
- Statement concerning future plans for the project
- Any supplemental statements or concerns
